SBAR Report Sheet for Nursing Students

Shift tool · Created by Professor Foxx, RN, MSN · Updated October 2026

This is the full SBAR report sheet Professor Foxx uses at the bedside. One sheet per patient: code status, isolation, PMH, tests, every body system, lines and drips, labs, drains, meds, and the recommendation. Print it, carry it on your clipboard, and fill it in as you take report. Nurses also call this a nurse report sheet, brain sheet, or SBAR handoff sheet. It is free to download as a PDF or Word file.

What is on the sheet

S · SITUATION

What is happening? When did this happen? Where did this happen? To be treated for (insert primary diagnosis). Plus name, room, age, gender, hospitalist or trauma, and code status (DNI / Full Code / DNR).

B · BACKGROUND

PMH checklist, tests (MRI, X-ray, CT, echo EF, endo, US, cath) with timelines, and admit date.

A · ASSESSMENT

IV site and access, IVF and drips, antibiotics, neuro and pain, respiratory and VS trend, cardiovascular and VTE prophylaxis, GI, GU, musculoskeletal, skin, BG monitoring, labs, drains, and medications. The sidebar tracks contact precautions, cultures and swabs needed, and extras like daily weight, strict I&Os, fall risk, 1:1 sitter, and NPO at midnight.

R · RECOMMENDATION

Always use a reference for your recommendation. Scheduled procedures, consults, and discharge plan.

Example: calling a provider

S: This is the nurse for Mr. Lee in room 12. I'm calling because his blood pressure dropped to 86/50 and he's newly confused.

B: He's 72, two days after a bowel resection, with a history of hypertension. His home lisinopril was restarted this morning. No known allergies. Full code.

A: HR is 118, RR 24, temp 38.6 °C (101.5 °F), SpO2 93% on room air. His abdomen is firm and tender, and urine output was 60 mL over the last 4 hours. I'm worried about sepsis.

R: I'd like you to come see him now. Can I get orders for blood cultures, a lactate, and an IV fluid bolus? I'll hold the lisinopril.
